The App
Community Health Survey v3.1 is a multi-institution, offline-first Progressive Web App (PWA) built for community health fieldwork. Powered by HazzinBR.

It was designed so that health workers can collect data in the field — even with zero internet — and generate official Ministry of Health format reports automatically. No paper, no spreadsheets, no stress. 🎉
The Developer
Atandi Brian — HazzinBR 👋
A developer who believes that technology should solve real problems for real people — not just look cool in a portfolio. This app was built with genuine care for public health in Kenya, with a love for clean code and the firm belief that community health workers deserve better tools than a clipboard and a prayer.

When not debugging JavaScript at 2am, Brian can be found wondering why CSS always has one more issue to fix. 😄

Can This App Be Adapted?
Absolutely yes. This app was built to be adaptable. It can be customised for any community health survey, any county, any university, or any public health programme in Kenya or beyond. Different survey sections, different questions, different report formats — all possible. Just reach out. The goal is better public health data for everyone. 🌍
Premium Platform
This app runs on a premium subscription model. Institution admins pay Ksh 500 to register their institution, and students pay Ksh 100 each to access surveys and generate reports. This keeps the platform maintained, fast, and always improving.

Quick Help
🔑 Login Issues
Use your admission number in the format given by your university (e.g. B11/GLUK/S53K/2022). Make sure you are connected to the internet the first time you register. If your access has been removed, contact your course coordinator.
📴 Working Offline
The app works fully offline. All your interviews are saved on your phone automatically. When you get back to an area with internet, tap Upload Data to sync your records to the server.
📑 Generating Reports
After finishing an interview, tap Finish and choose your report type. For all your reports together, tap My Reports on the home page. To print as PDF, open the report then use your browser's Share → Print → Save as PDF.
🔄 App Not Updating?
If the app looks different from what others have, close it completely and reopen. The app updates itself in the background when you have internet. If still stuck, go to your browser settings → Clear site data → reopen.

Acceptance
By registering and using the Community Health Survey application, you agree to the following terms. These terms are set by your institution's administrator in partnership with HazzinBR.
Authorised Use
This application is provided for use by registered users and authorised personnel of participating institutions for the purpose of community health situation analysis fieldwork. Use of the app for any other purpose is not permitted.
Data & Confidentiality
All data collected through this app is strictly confidential. Respondents must provide verbal informed consent before any interview begins. No names or personal identifiers of respondents shall be recorded. Data is stored securely and used solely for academic and public health purposes.
Data Accuracy
Each student interviewer is personally responsible for the accuracy and integrity of data they collect. Falsifying, fabricating, or manipulating survey data is a serious academic and ethical violation and may result in disciplinary action.
Account Responsibility
Your admission number and account are personal and non-transferable. Do not share your login details. You are responsible for all activity conducted under your account. Report any unauthorised access to your course coordinator immediately.
Access Control
The university and hospital reserve the right to revoke access to any user who violates these terms, misuses the application, or fails to comply with the ethical guidelines of the fieldwork programme.
Intellectual Property
The Community Health Survey application was developed by Atandi Brian (HazzinBR) and is provided free of charge. The application code, design, and reports remain the intellectual property of the developer. Redistribution or commercial use without permission is not allowed.
Disclaimer
This application is provided "as is" for academic and public health purposes. The developer and the university are not liable for data loss due to device failure, loss of internet connectivity, or other technical factors outside their control. Students are advised to sync their data regularly.
Updates to Terms
These terms may be updated from time to time. Continued use of the application constitutes acceptance of any revised terms. Last updated: March 2026.
